Case
62 – year-old women, with history of 5 years of diabetes presented with multiple painful necrotic skin lesions over legs, back and buttock with swelling off both legs 7-10 days. There was history of fever for 3-4 days. This nodular lesion had progressed to necrotic purulent discharge.
The lesions were necrotic with demarcated violet margins- associated with perianal involvement.
Her white cell count was high- renal function normal. Skin biopsy was done. Swab from lesions seen for – gram stain/AFB stain / pas/ and culture. Pus culture showed staphylococcus aureus (mssa) - appropriate antibiotic (meropenem /clindamycin/) started.
