Abstract

ABSTRACT-

Periodontal disease is a chronic inflammatory disease affecting the tooth supporting tissues and caused by gram negative anaerobic microorganisms. Preeclampsia is a pregnancy specific disease characterized by hypertension and proteinuria which affects 5-10% of pregnancies causing maternal & perinatal morbidity which can lead to mortality. This is due to infection and inflammatory responses. This article probes the relationship of preeclampsia with periodontitis and the various risk factors and mediators. Special lights arethrown into the pathogenic mechanism involved in preeclampsia highlighting the genetic and immunologicalaspects. The various protective and safer treatment modalities for preeclamptic women have also been discussed. Hence the article brings to light possible links between preeclampsia and periodontitis and evidences of the various treatment modalities being used till date.

Title: A Study of Nature, Magnitude and Pattern of Domestic Violence in Psychotic and Non Psychotic Married Women

Violence against women is a major public health and human rights issue in the world today. The problem of domestic violence is particularly serious in Asian Countries especially in India. Domestic violence, social and legal concept that, in the broadest sense, refers to any abuse—including physical, emotional, sexual, or financial—between intimate partners, often living in the same household. AIMS In this study we assess the prevalence and frequency of different forms of physical sexual emotional and economical violence against women. To find out whether the frequency domestic violence differs psychosis vs non-psychosis groups MATERIAL AND METHODs:-The study sample comprised of  50 women is the age group of 16 to 40 year with Axis 1 disorder as per DSMN 4-TR. The sample was selected from the Department of psychiatry OPD of SS Hospital, BHU. In this study we use four types of questionnaire i.e. Domestic Violence questionnaire-1, Violence questionnaire-2. The questionnaire was used for identifying the various characteristics of domestic violence among females with mental illness. RESULTS:The finding of study Domestic Violence was observed in 64% of sample. In the subjects all categories of domestic violence like verbal violence66%, 48%, physical violence 46%, and economical violence 18%, sexual violence 14% dowry 8% and stridhan related violence 4% were present. In verbal violence ridicule and In physical violence slapping was more common , whereas 30% of sample was psychotic and 20% of sample non-psychotic Husbands were mostly responsible for violence in majority of cases and some women reported the involvement of husband’s parents.

Abstract

ABSTRACT:

 Basal Cell Carcinoma (BCC) frequently involves sun exposed sites of the face such as the nose, eyelids and peri-orbital tissue. An adequate surgical excision is still the gold standard for treatment. Deep infiltration and involvement of the orbit by periocular BCC is rare but may be clinically silent and thus should be looked for on imaging especially in recurrent and high grade variants. Despite the fact that orbital exenteration is one of the most socially distressing facial operations, it may still be necessary.  The repair of a radically exenterated orbit is a difficult problem for surgeons to resolve. We present the case of a locally advanced re-recurrent periocular BCC treated with a suprastructural maxillectomy combined with an orbital exenteration and ethmoidectomy; reconstruction with a free Transversus Rectus Abdominous Myocutaneous flap and adjuvant post-operative radiotherapy.

Title: Gender Differences in Cardiovascular Responses to Isometric Exercises in Normotensive Subjects

Abstract

ABSTRACT

The study is aimed to determine the gender differences in cardiovascular responses to isometric exercise. 25 normotensive females and 25 normotensive males (age – 20-25 years) were made to perform handgrip exercise. The following parameters were studied – i) Maximum voluntary contraction (MVC), ii) Endurance time (ET), iii) Heart rate, systolic blood pressure, diastolic blood pressure and cardiac work was measured before and immediately after the handgrip exercise at 40% MVC. iv) all the above parameters were compared in between males and females. Results-1) MVC was significantly high in males as compared to females. 2) Endurance time was found significantly high in females as compared to males. 3) The heart rate, systolic blood pressure and cardiac work increased significantly after hand grip exercise in both, males and females as compared to before exercise. Sympathetic activity is accelerated after isometric type of exercise. The responses were greater in males as compared to females.

Title: A study of variation in Heart Rate Variability with change in posture in young adult Indian males

Abstract

Heart rate variability is predominantly dependent on the extrinsic regulation of the heart rate. The HRV response to postural change is a sensitive measure of the shift in autonomic balance from parasympathetic to sympathetic predominance that, when attenuated or absent, has been correlated with prevalent disease in patient populations. Hence the aim of the study was 1. To study the effect of change in posture on HRV in young adult Indian males. 2. To compare HRV parameters in different postures in males. The study was conducted on 50 young adult males. Their Height, Weight, Pulse rate & BP were noted. HRV was recorded with the help of Anu photo rheograph using frequency domain method in supine, sitting and standing positions. The data was analysed using SPSS 17.0 statistical package. Comparison between groups (supine, sitting and standing) was done using one way ANOVA and Tuckey’s POST HOC test. The statistical significance level was established at 5% (P < 0.05) and 1% (P < 0.01). The mean RR Interval and mean high frequency for males in supine, sitting and standing  positions was 0.82 ± 0.15, 0.78 ± 0.12, 0.69 ± 0.10 and  20.00 ± 11.30, 14.20 ± 8.76, 13.32 ± 7.31 respectively [significantly different (P = 0.00 and 0.001 respectively)] Both decreased when posture changed from supine to sitting to standing. The mean low frequency for males in lying, sitting and standing positions was 27.76 ± 8.40, 34.28 ± 9.08 and 30.73 ± 8.87 respectively [significantly different (P =0.001)]. It increased when posture changed from supine to standing to sitting. Our study shows that HRV components - mean RR interval, mean LF and mean HF are significantly different with posture change from supine to sitting to standing in young adult males. These findings correlate with decreased parasympathetic and increased sympathetic influence with postural changes from lying to sitting to standing. Heart Rate Variability parameters such as mean R-R interval, mean LF and mean HF showed significant difference with variation in posture from supine to sitting to standing in young adult males. This correlates with decrease of parasympathetic tone and increase of sympathetic influence with postural changes from lying to sitting and to standing positions.

Title: Morphometric Study of Tibial Condylar Area in the North Indian Population.

The upper end of tibia is expanded to form a mass that consists of two parts:  lateral and medial condyles which articulate with the corresponding condylar surfaces of the femur. Separating these two condyles is the intercondylar area whose central part is raised to form the intercondylar eminence. The present study will give information of the exact dimensions and percentage covered by medial and lateral condyles out of total condylar area. This study was undertaken to collect metrical data about the medial and lateral condyles of tibia. The present study was performed on 150 dry tibia of north Indian subjects, Out of which 70 tibia belonged to right side and 80 were of left side. The age and sex of these bones were not known. The anteroposterior length of medial and lateral tibial condylar area was measured along with their transverse diameter. The data was statistically analyzed to hold comparisons between tibia of right and left side and also between medial and lateral tibial condyles of the same side. The area covered by MTC is 38.56% and by LTC is 35.97% out of total condylar area in right side. The area covered by MTC is 37.32% and by LTC is 35.65% out of total conylar area in the left side. The study is important for anatomists, anthropologists, and orthopedics regarding unicompartmental knee arthroplasty (UKA), complete knee arthroplasty procedures and meniscal transplantation.

Title: Transintestina Lmigration of Needle -Rare Case of Foreign Body in Kidney

Foreign bodies in the kidney are rare. We report a rare case of trans-intestinal migration of a sewing needle into the right kidney in a 23 year old female who was admitted for intermittent pain in the right flank for 3 years. Patient had history of swallowing a needle accidentally 3 years ago, and was diagnosed to have the same located within the Right renal parenchyma. Radiography revealed a linear metallic shadow in the right kidney. CT scan confirmed it to be in the right renal parenchyma. The foreign body was extracted operatively.

Title: The Use of Buccal Fat Pad Reconstruction in Oral Sub mucous Fibrosis

Abstract

An oral fibrosing disease was first described in the year 1952 by Schwartz which was later termed as “Oral Submucucous Fibrosis” by Joshi in the year 1953. The management of this disease can be of two categories: medical and surgical. The buccal fat pad has been an easy and effective method in the surgical management of oral submucous fibrosis and is discussed here. The buccal fat pad is epithelialized within 3 to 4 weeks and hence further skin grafts are not required. The review was done by a websearch of case reports in using buccal fat pad for the management of oral submucous fibrosis. Studies suggest that the use of buccal fat pad is a better choice of treatment for managing oral submucous fibrosis. The easy way of use, quick healing and rich vascularity provides better function and aesthetics and thus seems to be an appropriate choice for the surgical treatment of oral submucous fibrosis.

Title Conservative Management of Cerebral Venous Angioma - A Difficult Alternative for Patient and Physician

Abstract

Cerebral venous angioma, also referred to as a developmental venous anomaly is a congenital anomaly of the medullary vein, the vessel that drains into the transparenchymal venous stem. We describe a 7 year old male child presenting with left sided focal seizures in status followed by left hemiplegia. On examination child had altered sensorium and left hemiplegia. His baseline blood, metabolic and CSF studies were normal. MRI brain and MR angiography detected venous angioma involving the deep white matter of Right frontoparietal lobe. Currently child is being managed conservatively on medical therapy as mentioned in the literature. This report describes the clinical and radiologic findings for large venous angioma that caused seizures in a child.

Abstract

In this report the author treated a patient of agoraphobia with Paroxetine, who subsequently developed galactorrhea. The lab reports had shown normal levels of serum prolactin levels in this case. Discontinuing paroxetine decreased the symptoms of galactorrhea in this patient. The study may establish a causal relationship between paroxetine and galactorrhea.

Title: Voluntary Organization and Its Challenges in India

Abstract

Non-governmental organizations (NGOs) are legally constituted organizations created by natural or legal people that operate independently from any form of government. The term originated from the United Nations, and normally refers to organizations that are not a part of a government and are not conventional for-profit businesses. In the cases in which NGOs are funded totally or partially by governments, the NGO maintains its non-governmental status by excluding government representatives from membership in the organization. The term is usually applied only to organizations that pursue wider social aims that have political aspects, but are not openly political organizations such as political parties.

The number of NGOs operating in the United States is estimated at 1.5 million. Russia has 2,77,000 NGOs.  India is estimated to have had around 3.3 million NGOs in 2009, just over one NGO per 400 Indians, and many times the number of primary schools and primary health centers in India.

Title: Detection of Latent Tuberculosis Infection in Nursing Students by Combined TST and IGRA Serial Testing

Abstract

Nursing students are  Occupationally at risk group for   tuberculosis. Tuberculosis can either be as Latent tuberculosis  (LTBI) or as active tuberculosis infection (TBI). Lack of awareness and not taking appropriate self defence exposes this group to acquire tuberculosis. it is the prime responsibility of governments and the managements of hospitals must take care of the health care workers.

There is no optimal or single specific test for detection of LTBI. Tuberculin skin test ( TST) is used for screening to detect LTBI. But TSThas many variations, modern immunological tests like detection of interferon gamma release assay are found to be more reliable. In TB high burden countries, persistently elevated interferon gamma level is found in some health care workers, constantly exposed to M.tuberculosis positive clinical samples. Serial testing of combined TST and Quantiferon gold helps in detecting nursing students with LTBI. Methodology ; 85 nursing students on admission were tested for detection of Latent TBI. In TST the induration size was increased to 15 mm.  these tests . 25 first year  medical students and fifteen microbiology techinicians were included as controls. Result : only one nursing student showed 15 mm induration and a very high Gamma interferon level, persistently positive gamma interferon was found in 5 technologists, combined test after three, six and twelve months overcame the false positives faced in single TST or IGRA
